End of life planning toolkit for people with a learning disability

The Victoria & Stuart Project created this toolkit with people with learning disabilities, their families, learning disability support staff, and healthcare professionals.

The toolkit has picture cards and guides to help you support people with a learning disability plan for the end of life. It also includes recording sheets, so you or someone you're supporting can write down and keep their thoughts and wishes.
